About Your Visit

PATIENT INFORMATION

Choosing to undergo a cosmetic procedure is an important decision, and our team of professional and caring staff are dedicated to helping you throughout this personal process. Your first consultation for a cosmetic procedure, there is a $80 fee. This fee is applied towards any treatments or services at the office of Dr. Tran.

Health insurance is accepted for other reconstructive or medical conditions. According to your insurance plan, a co-pay will be collected, and there may be other up-front costs depending on your deductible. All payments will be collected at your initial visit prior to seeing the physician. We encourage patients to call their insurance company to learn more about their policy and out of pocket payments. Your initial consultation will be billed through your health insurance, but this does not mean that your insurance company will cover the procedure. Our office will work hard to see if your insurance company will provide authorization for the procedure after your consultation. If you do not have health insurance, there is a consultation fee for your visit.

 

Reconstructive/Medical Conditions Include:

  • Keloid, hypertrophic, or acne scars
  • Scar revision
  • Lesion or mole removal
  • Skin cancer reconstruction
  • Rhinophyma
  • Facial deformities or paralysis
  • Facial trauma
  • Broken nose
  • Deviated septum

 

Appointment Cancellation Policy:

At the office of Dr. Anthony Tran, we take pride in delivering quality and personalized care. Each and every appointment space is important, and we do not overbook consultations. If it is necessary to cancel your scheduled appointment, we require that you call or email us at least 24 hours in advance, and calling early in the day is appreciated. If a patient does not provide a 24 hour notice of appointment cancellation, then there will be a fee of $100 to schedule their next appointment. This fee is non-refundable, but may be applied to surgery or other services provided at the office of Dr. Tran.

About Your Visit

In Preparation for Your Visit:

Please let us know if you would like to receive our new patient paperwork via mail or email. Completing forms in advance will help expedite your visit. Please bring any pertinent medical records or diagnostic tests, if applicable. Many patients like to bring their own written questions and concerns to discuss during the visit. A copy of our notice of privacy practices is available in our office.

Upon Arrival:

You will be greeted by one of our friendly Patient Care Coordinators, who is available to answer any preliminary questions you may have. We value patient privacy, and schedule ample time in between patients for your convenience and discretion. We ask that you please contact us more than 24 hours in advance if you need to cancel or change your appointment.

During Your Consultation:

You can expect a comprehensive, personalized consultation with Dr. Tran. He will discuss your concerns with you and complete a physical examination. An individualized treatment plan will be developed. Dr. Tran will discuss any of the proposed treatment options with you, to help you decide which treatment plan is right for you.

After Your Consultation

After the consultation, you will meet with one of our Patient Care Coordinators, where cost of the procedure and financing options are discussed. Your visit will be in a comfortable, no pressure atmosphere to schedule a treatment. We support your personal choice in selecting the surgeon that you feel comfortable and confident with.

Scheduling Your Surgery

Dr. Tran requires two visits before undergoing any surgical procedure. Once you have decided to proceed with surgery, our staff makes the scheduling process as smooth as possible.

 

SURGERY FINANCIAL POLICY

Deposit

  • A $500 deposit is required to book surgery and reserve your surgical date.
  • The deposit is non-refundable, but it can be transferable to other services offered at Anthony Tran.MD Cosmetic and Plastic Surgery Center.
  • The deposit is valid for one-year from the date of payment. The value of the deposit will expire after a period of one year, and the deposit will be lost.

Surgery Balance

  • The remaining surgery balance is due at least 14 days prior to the surgery date.
  • Acceptable forms of payment are: Cash, Credit/ Debit Card, Cashier’s Check, Money Order, or Care Credit Financing.

Rescheduling of Surgery

  • To reschedule your surgery, please notify us at least 21 days in advance.
  • Your deposit will be applied to your rescheduled surgery date, as long as it occurs within one year of the date the deposit was given.
  • If surgery cannot be rescheduled within one year, the $500 deposit can be used for other services.

Surgery Cancellation

  • If surgery is cancelled more than 21 days prior to the surgery, you will be refunded all fees, expect for the non-refundable $500 deposit, which is transferable to other services.
  • If you cancel between 7-21 days before surgery, all fees will be refunded EXCEPT:
    • One half (1/2) of the surgeon’s fee.
    • Non-refundable $500 deposit (This is transferable to other services)
  • If cancellation occurs less than 7 days before the scheduled surgery, all fees will be refunded EXCEPT:
    • The entire surgeon’s fee
    • Non-refundable $500 deposit (This is transferable to other services)
  • Should the physician cancel your surgery for any reason, you are entitled to a full refund (including your deposit) if you cannot reschedule.
